The system creates clear lines of accountability: who authorised the work, who planned it, who is supervising it, and who is watching over the high-risk elements. When those lines blur, people get hurt.<\/p>\n\n\n\n<p>There are two primary roles in every permit-to-work system: the Permit Issuer and the Permit Receiver. Alongside these sit dedicated monitoring roles such as Safety Observers, and management positions with broader oversight responsibilities. Each has distinct obligations, and none of them are optional.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Permit Issuer<\/h2>\n\n\n\n<p>The Permit Issuer is the person who authorises high-risk work. If you are <a href=\"https:\/\/www.drivingtests.co.nz\/course\/permit-issuer-training\/\">training to be a Permit Issuer<\/a>, this is your role. Your job is to <a href=\"https:\/\/www.drivingtests.co.nz\/resources\/hazard-identification-and-risk-assessment-for-permit-issuers\/\">confirm that the hazards have been identified<\/a>, the risk assessment is adequate, the controls are in place, and the people doing the work understand what they are walking into. You review the scope of the work, inspect the worksite, verify isolations, and confirm that all supporting documentation is complete before you sign the permit.<\/p>\n\n\n\n<p>You are the central coordinator for all permitted work on your site. You need to maintain an overview of what work is happening, where it is happening, and whether any of it could conflict with other activities. Consider a site where one team has a permit to drain a process line on the second floor while another team is doing maintenance on pipework directly below. If neither permit references the other, and nobody has checked whether the drain discharge could affect the lower work area, the coordination has failed. That failure sits with the Permit Issuer.<\/p>\n\n\n\n<p>You have the authority to reject or postpone any work. If the risk assessment is inadequate, the controls are not in place, the people are not competent, or site conditions have changed, you do not issue the permit. This is not a discretionary power you exercise occasionally; it is the foundation of the entire system. A Permit Issuer who signs off on work they are not satisfied with has failed in their role.<\/p>\n\n\n\n<p>That authority will be tested. A contractor will turn up with a crew on the clock and a project manager on the phone asking why the permit has not been issued. You will have to hold your position, because the consequences of issuing a permit against your better judgment are always worse than the delay.<\/p>\n\n\n\n<p>A Permit Issuer must be formally appointed by the organisation. Completing a training course is not enough. Your organisation must authorise you to issue permits for the specific site or plant you will be working on, maintain a register of authorised issuers, and review that authorisation periodically. <\/p>\n\n\n\n<p>Where more than one Permit Issuer operates on a site, they must coordinate so that each issuer has current information about all active permits. At shift changeover, the outgoing issuer hands over with a full briefing on active permits, their status, and any concerns.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">No self-permitting<\/h2>\n\n\n\n<p>One rule applies without exception in every permit-to-work system: a Permit Issuer cannot self-issue a permit. You cannot be both the person authorising the work and the person carrying it out. The entire point of the system is independent verification. If one person plays both roles, there is no independent check on the hazard identification, the risk assessment, or the adequacy of controls. This prohibition exists in every PTW procedure for good reason, and breaching it is treated as serious misconduct.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Permit Receiver<\/h2>\n\n\n\n<p>The Permit Receiver is responsible for planning the work, preparing the risk assessment or Job Safety Analysis (JSA), and submitting the permit application with enough time for proper review. For planned work, that is typically at least 24 hours&#8217; notice; for major project work, significantly more.<\/p>\n\n\n\n<p>In many organisations, the Permit Receiver role is split into two functions. The Permit Applicant plans the work and prepares the documentation. The Person in Charge of the Worksite (PICWS) supervises the execution on the ground. In smaller operations, these may be the same person. Either way, the Permit Receiver must understand the scope of the job, the hazards involved, and the controls required. When you issue the permit, the Permit Receiver signs to confirm they understand and accept the conditions. That signature is not a formality; it is acceptance of responsibility.<\/p>\n\n\n\n<p>Where the PICWS is a separate role, they are on the ground with the work team and are accountable for making sure permit conditions are followed throughout the job. The PICWS must brief all workers on the hazards, controls, and emergency procedures before work starts. If the scope changes or new hazards appear, the PICWS must stop work and notify the Permit Issuer. Work does not continue under a permit that no longer reflects actual conditions. One is most commonly required for confined space entry, hot work, working at height where fall arrest is in use, excavation, and lifting operations. Your organisation&#8217;s procedures will define when one is mandatory.<\/p>\n\n\n\n<p>The critical point about a Safety Observer is that they observe. They do not participate in the work or carry out any other duties while on watch. Their sole job is to monitor the work, maintain communication with the people in the hazard zone, and initiate emergency action if something goes wrong. If they need to leave their post, a replacement must be in position before they step away. If no replacement is available, the work stops.<\/p>\n\n\n\n<p>Consider a confined space entry where the Safety Observer is also asked to move materials to the entry point while monitoring the person inside. If the person inside collapses from oxygen depletion while the observer is fifty metres away collecting equipment, the observer cannot do their job. That is how people die in confined spaces. The role must be dedicated and uninterrupted.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Management and oversight roles<\/h2>\n\n\n\n<p>Above the operational roles, your organisation will have management positions with specific PTW responsibilities. The Permit System Manager is responsible for making sure the PTW procedure is documented, approved, implemented, and regularly reviewed. Site managers or plant leads are accountable for the operation of the system on their site, including making sure there are enough trained personnel to run it.<\/p>\n\n\n\n<p>People leaders must make sure their staff and contractors understand and comply with the permit to work system. These are obligations under the Health and Safety at Work Act 2015, not optional best practice.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Stop work authority<\/h2>\n\n\n\n<p>Every person on site, regardless of their role, has the authority to stop work if they believe it is unsafe. If a concern cannot be resolved through discussion, the work must be stopped and the permit withdrawn until the Permit Issuer is satisfied that conditions are safe.
